Dane County 4-H Community Service Survey

Dane County UW-Extension 4-H Youth Development staff and the Dane County 4-H Leaders Association are interested in what clubs are doing for community service activities. We are conducting a survey to gather information about the scope and impact of club community service activities.

With the information collected from this survey, the Leaders Association will collaborate with staff to:

  • Compile community service activity ideas into a resource to share with all clubs in Dane County.
  • Develop resources to support clubs in enhancing these activities to incorporate service learning.
  • Communicate information about the impact that 4-H clubs are making in communities to elected officials, potential donors, and other community stakeholders.

Please forward this survey to the contact from your club who is most knowledgeable about the service project. This could be the club organizational leader, an adult or youth chair of a community service committee, or a youth club officer. If your club completes multiple community service projects in one year, you can submit the survey multiple times. Please describe one project at a time when completing the survey.

Please submit survey responses by May 31, 2018.
